Sliding Fee at Cleveland Ave Dental Center

Starting July 1, Cleveland Avenue Dental Center, part of the Forsyth County Department of Public Health, will be implementing a sliding fee schedule. All self-pay patients may qualify. To qualify, an application must be completed and all household income must be verified. Accepted forms of income include: W2 forms, check stubs, employer verification forms, Social Security income, etc. To find out more and to see if you qualify, call the Cleveland Avenue Dental Center at 336-703-3090.
